Terms and conditions for Stroller Strides Unlimited Monthly Membership

  • This contract is made between the client and FIT4MOM Southwest Denver and is a minimum 3 month agreement. It may be terminated by either party with the proper advance notice. This membership allows you to attend unlimited Stroller Strides classes.

    AUTHORIZATION FOR PAYMENT
    I authorize FIT4MOM Southwest Denver to initiate a charge to the account entered into Front Desk for my dues. This authorization is to remain effective until I have cancelled my membership through the FIT4MOM Southwest Denver administrator. The purchase date is the future billing date. Ex: if you purchase your membership on the 16th of the month, the dues are paid in full for that month and the next charge will be the 16th of the next calendar month.

    I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Southwest Denver to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on file on the day of the month when the membership was started. The processing date for debit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend class who have unpaid balances. If a membership remains unpaid it will be cancelled.

    CANCELLATION POLICY
    To cancel this membership, the member must contact the FIT4MOM Southwest Denver administrator prior to their next payment due date to request a cancellation. No refunds will be given for dues once it has been paid. Notification of cancellation must be emailed to hannah.nickle@fit4mom.com at a minimum of 30 days before the requested date of cancellation. If a 30 day notice to cancel is not provided a $25 cancellation fee will be charged. This membership is not transferable to another person. Once a membership is cancelled, a member can re-enroll within 3 months with no registration fee. If the client re-enrolls after 3 months from cancellation, half of the registration fee will be charged.

    HOLD POLICY
    Members may put their account on hold for a $10 hold fee. Please email hannah.nickle@fit4mom.com to schedule an account hold.

    By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with proper advance written notice.
